If you would like to submit your return via mail, affix the physical sticker to a printed copy of this completed form and mail it to:

If your Portacool was manufactured before 2020 the date code is located in the top right corner of the ID plate on the outside of the cooler. If your Portacool was manufactured after 2020, the date code is in the first 5 digits of the serial number.
Your cooler’s serial number is a series of all digits located on a metal plate on the outside of your evaporative cooler.
